Forehead has important role to influence personal impression. The image could be different by shape or position of forehead. If your hairline is up to high that makes your forehead large, you could be looked long and big face that makes coarse impression. The golden ratio of the face is that the distance from the tip of the chin to the tip of the nose is 0.9, the distance from the tip of the nose to the forehead (between eyebrows) is 1, and the distance from the forehead to the hairline is 1. More than 90% of patients who want to slightly lower the center of the hairline and fill the M-shaped part are satisfied. If the forehead is too large and the occipital hair is not enough to have hair transplant, forehead reduction surgery may be a good alternative method. Occasionally, scalp tattoos are made. Hair transplant for hairline correction is divided into FUT (Follicular unit transplantation) and FUE(follicular unit extraction). FUT is that doctor cut strip of skin from the occipital region scalp and follicles are removed from the strip, and then re-inserted into hairline area. FUE is that doctor collect follicles one by one with a punch on the occipital region after shaving hair in the area and re-insert it to the hairline area. FUT allows for a large-scale transplant but the scar on scalp remains. FUE is less painful, but it is difficult to collect a large number of follicles compared to FUT. Also, the graft survival rate is different between FUT and FUE.
Dr. Hwang explained, “FUT has better graft survival rate then FUE. People choose the hairline correction for cosmetic satisfaction, but if the graft survival rate is low, they may not be satisfied. If it is possible, hairline should perform in FUT for the satisfactory results. If you are concerned about the discomfort caused by scars or incisions, you can choose FUE but it should be aware of the graft survival rate may low.”
Natural result of hairline correction depends on how taking care of fine hair. There are one or more hairs in one hair follicles, the fine single hair follicle should be transplanted to the frontmost hairline to get a natural result.
Especially, after removing the hairline, it often looks awkward. It can be unnatural if you don’t save fine hair by focusing only on large forehead. Usually, there is fine hair in the front of the hairline, and the hair gets thicker as it goes inward. If it has become unnatural after hairline removal, it is a recommended way to make it natural by transplanting fine single hair follicles in the front area.
Most of the transplanted hair falls out within a month, but it starts to grow again 4 to 5 months after the surgery. It grows by 1 to 1.5 centimeter per month, and after a year or so, it is in a healthy state like the surrounding hair.
