As you get older, small bumps may appear around your eyes, cheeks, or neck. These are ‘Syriongoma’, ‘Milia’, and ‘Skin tag.’ Most people think of it as a pimple and try to remove it by squeezing it out, but it is easy to cause a big problem if you treat it incorrectly, so proper treatment is necessary.

Syriongoma is a benign sweat gland tumor caused by abnormal proliferation of sweat glands. It begins to appear in women in their 30s and 40s, and the number increases with age. It mainly appears in the area under the eyes, but it can also appear on the cheeks, forehead, armpits, abdomen, etc. Rarely, it appears in the groin or around the genitals. 

In the beginning, it stars as small, skin-colored, or reddish-brown papules that are unrecognizable, the grows larger over time, merges with other syriongomas, and spreads to the surrounding area. Syriongomas are systematically located deep in the skin and the surrounding area is fibrous, so it is difficult to treat and there are many cases of recurrence even treatment due to the influence of heredity. 

Milia is a keratin cyst that is caused by the accumulation of sebum in the pores due to poor secretion of sebum. It looks like a small yellow rice grain-like pouch filled with dead skin cells. 

Milia can be divided into ‘primary milia’ and ‘secondary milia’ according to the cause. Primary milia is a naturally appearing milia without any specific cause, and it appears frequently on the cheeks and eyelids. Secondary milia appears at the area suffering from a skin disease or after the skin has been damaged by burns or dermabrasion. Primary milia can improve spontaneously, but secondary milia do not improve spontaneously so it is required to treat in the clinic. 

Skin tag is also called soft fibroma. It is a benign tumor that appears due to abnormally enlarged skin fibers. The exact cause is not known, but it is known that there is a certain amount of genetic influence, and it begins to appear as the skin ages. It mainly appears as skin-colored or brown papules of 1-2mm or several centimeters in the folds such as the neck, armpits, and groin.

Syriongomas, milia, and skin tags are simply cosmetic problems and do not immediately adversely affect health. However, it is difficult to clearly distinguish visually from other diseases such as warts, age spots, and comedonal acne. If it is mistaken for a simple pimple and is treated incorrectly, it can make symptoms worse. 

Some people treat it a variety of folk remedies such as clipping with nail clippers or periodically applying vinegar or glacial acetic acid, but it should be careful as it can cause serious problems.

Dr. Ha Eun Lee, a founder of Foret Dermatology Clinic, said, “If the secondary bacterial infection occurs while a lesion is cut with a nail clipper and a secondary bacterial infection occurs, the skin may become red and irritated, and finally, there may be cases where you have to take medicine. In the case of vinegar or glacial acetic acid, if applied incorrectly to thin skin such as under the eyes, it can cause chemical burns and pigmentation, and in severe cases, it can leave scars. If syriongomas, milia and skin tags appear, you should not touch it by yourself. It is important to get proper treatment.”

Treatment of milia and skin tag is relatively simple. For the treatment of milia, we can use a Co2 laser or needle to pierce the lesion and extrude the sebum inside. The skin tag can also be easily removed by burning it with a Co2 laser. 

However, it is difficult to treat syriongoma because it is histologically located deep inside the skin and the surrounding is fibrous. Even after removing by laser, the recurrence rate is high. Thus, rather than overdoing it all at once, it is better to treat several procedures in a complex manner and divide it several times for regular treatment. 

Dr. Lee explained, “If you think that you can get rid of syriongoma at once, it is easy for the patient to get stressed. So, I advise for my patient that treating it once a year, when syriongoma comes up. If it is treated regularly, the frequency of occurrence is reduced, and even if it appears as the skin elasticity improves through repeated treatments, it does not appear severely. If it is completely removed, there is a high possibility that it will recur in another area, so removing it once a year regularly is one way to do it.”

In the past, all syriongoma was treated with a Co2 laser, so there was a risk of pigmentation or scarring, and the recovery period took a long time. Recently, there is a new method, ‘Pinhole’ that reduces the risk of scarring and recovery is quicker. The ‘Pinhole’ is a method of removing the inside of syriongoma by irradiating the high energy as if drilling a hole with a Co2 laser after removing the surface of the syriongoma with an erbium laser. In some cases, fractional laser can be used to regenerate the skin and improve the elasticity of the surrounding skin. 

You can prevent syriongoma, milia, and skin tag by applying a moisturizer and sunscreen every day. It is best to avoid rubbing your eyes excessively or using high-functional cosmetics for a long time, as it may irritate them. Above all, it is better to treat quickly before the lesion grows or the number increases. 

Dr. Lee advised, “Syriongoma, milia and skin tag can be prevented through methods that can prevent skin aging, such as applying a moisturizer and sunscreen well. In the case of milia, it often appears when there is a skin disease, so if you have itching or dermatitis, treat it immediately. It is recommended not to irritate the eye area and, to treat it quickly when the number is small.”
